When calling your insurance company, we encourage you to ask the following questions, to help determine your coverage.

please call the customer service number on the back of your card, prior to your first visit.

  1. Is Renew Nutrition Therapy (NPI number 1902276520) currently an in-network provider for my plan?
  2. If not, what are my out-of-network benefits?
  3. Does my plan cover outpatient nutrition therapy CPT codes 97802 (medical nutrition therapy – assessment), 97803 (medical nutrition therapy – follow up) and/or 97904 (medical nutrition therapy – group)?
  4. Do I have any nutrition counseling visits covered under the preventative care portion of my plan, (ICD-10 code z71.3)?

    Note: in the event that you also have a medical diagnosis (determined by your medical provider- ex: diabetes), please ask if any/all of those relevant ICD-10 codes may also be covered.
  5. If I have telehealth coverage, is there a particular telehealth platform that is required?
  6. Do I need a physician referral?
  7. Do I have a deductible for “Nutritional Counseling Services?
  8. If yes, how much is it and how much has been met so far?
  9. Is there a copay for each visit or do I have co-insurance? If I have co-insurance, what is the percentage of coverage?
  10. How many sessions are covered per year?
  11. What month does the policy year renew?
  12. Are there any restrictions and/or limitations to my coverage?
  13. What is my reference number for this call?
